If you want a free printable calendar template that I have made that you can print in a 12 x 12 format use this Download Calendar_2009_01  Be sure to indicate when printing that you want it to be 12 x 12 . Combine this with 12 x 12 PDQ pages and then take it to Staples or Kinkos to have it bound. It makes a great gift! I will post photos of mine once I have it finished.
